Payment step

Input: CareActSettlement Output: ReimbursementPayment and HospitalPayment

This step also creates other legacy objects (CareCase, etc).

Note that the entrypoint for this step is a not a CAS ID, but a claim source ID (Document / BVACFile / HospitalizationInvoice).
